Heart Failure

Each patient placed into Mission Home Care’s cardiac program will be given their own self-care kit which includes:

• Heavy duty talking scale
• Scanning thermometer
• Easy to use pulse oximeter for determining oxygen levels
• Automated blood pressure recorder

Patient self-use of these tools allows for a more constant monitoring the heart’s status even when the Mission Home Care clinicians are not in the home.

Before beginning this program, each patient is educated on the importance of self-monitoring and how to obtain and report the results.

Behind the scenes, we monitor these results in order to make necessary adjustments to treatment.
