What is home health care?

Home health care is skilled medical care provided in the comfort of your own home. It includes services like skilled nursing, physical therapy, occupational therapy, speech therapy, and more — all delivered by licensed professionals who come directly to you. Please note: we provide skilled home health care and are not an hourly service. We do not provide around-the-clock or continuous custodial care. If that level of support is what you need, our staff is happy to point you in the right direction and provide recommendations for unskilled home care providers in the area.

Who qualifies for home health care?

Patients who are homebound (meaning leaving home requires considerable effort) and who have a skilled care need ordered by a physician typically qualify for home health care. This includes patients recovering from surgery, managing chronic conditions, or needing rehabilitation after a hospital stay.

Question 3

How do I get started with Mission Home Care?

An initial order for home health care must come from the patient's primary care physician (PCP) or as a referral from a hospital. Patients have the right to choose which home health agency they would like to use for their care. Once a referral is received, you can reach us at 813-355-4804 or email intake@missionhc.net and our care coordinators will guide you through the rest of the process.

What does CHAP Accredited mean?

CHAP stands for Community Health Accreditation Partner. CHAP accreditation is a nationally recognized seal of approval that demonstrates a home health agency meets the highest standards of care, safety, and quality. Mission Home Care is proudly CHAP accredited, which means we have been independently evaluated and verified to deliver care that meets or exceeds Medicare Conditions of Participation. For patients and families, CHAP accreditation means you can trust that the care you receive is held to a rigorous, third-party standard — not just what is minimally required by law.

Is Mission Home Care available 24/7?

Our phone line is available 24 hours a day, 7 days a week for urgent needs. Our office hours are Monday through Friday, 8:30am to 5:00pm. After-hours clinical support is available for active patients.

Question 6

What insurance plans do you accept?

Accepted Answer

Mission Home Care accepts Medicare and several major commercial insurance plans. We are a Humana contracted and preferred provider, accepting Humana Commercial PPO, Commercial HMO, Medicare Network PFFS, Medicare HMO, and Traditional Plans. We also accept United Healthcare PPO. We are also Community Care Network providers with the VA, meaning eligible veterans can receive home health services through their VA benefits. If you do not see your plan listed, please call us at 813-355-4804 — we are happy to work with you to determine if we can accept your coverage.

Do you accept Medicaid?

Do you serve veterans through the VA?

Yes! Mission Home Care is a proud partner of the VA Community Care Network. Eligible veterans can receive home health services through their VA benefits. Contact us and we will coordinate directly with the VA on your behalf.

What areas do you serve?

We serve East Pasco County and surrounding communities, including Zephyrhills, Dade City, Wesley Chapel, San Antonio, Land O Lakes, and Lutz. Please call us at 813-355-4804 to confirm service availability in your specific area.

How quickly can care begin?

In many cases, we can begin services within 24–48 hours of receiving a physician's order and completing the intake process. Urgent situations may be accommodated even faster.

Who will be providing my care?

All care is provided by licensed, credentialed professionals — registered nurses, licensed practical nurses, physical therapists, occupational therapists, speech-language pathologists, and certified home health aides. All staff undergo background checks and ongoing training.

Can I request a specific caregiver?

We do our best to maintain consistency in your care team. While we cannot always guarantee a specific caregiver due to scheduling, we strive to minimize the number of different clinicians visiting your home.

What is your hospitalization reduction program?

Our hospitalization-reduction initiative is a proactive care model focused on preventing unnecessary hospital readmissions. It includes close monitoring, medication management, early detection of warning signs, and strong communication with your physician — all designed to keep you healthy and at home.
